New Patients: Intake Appointments for Behavioral Health Services
If you are a new patient looking to make an appointment with a social worker or psychiatrist at the Gregory P. Shadek Behavioral Care Center at Englewood Health, the first step is a short intake assessment. These assessments are conducted by telephone.
Use our online self-scheduling system below to schedule your intake assessment. Our intake team may then refer you to an appropriate social worker or psychiatrist for psychotherapy and/or medical treatment.
There is no charge for these calls, but you will be asked to provide your insurance information.
